We support our team with a competitive compensation and benefits package, including:
- Hourly compensation between $20.50 - $21.35, PLUS an annual performance-based, discretionary incentive.

What You Will Do - Essential Functions The Care Coordinator supports the chronic care management team by coordinating non-clinical care activities, providing essential administrative support, and ensuring accurate documentation and communication to enable efficient, high-quality, and patient-centered care delivery.
- Non-Clinical Care Coordination & Patient Support
- Coordinate non-clinical components of patient care plans, including referrals, DME orders, appointment scheduling, and follow-up activities.
- Serve as a primary resource for patients and families for routine questions, ensuring timely escalation of clinical issues to the appropriate care team members.
- Administrative & Operational Support for Care Delivery
- Act as a first point of contact by managing incoming phone calls, faxes, and correspondence, and routing inquiries appropriately.
- Provide administrative and workflow support to the care team and organization, including special projects and tasks that enhance program effectiveness and patient outcomes.
- Data, Documentation & Records Management
- Facilitate the receipt, organization, and reconciliation of external medical records critical to patient care.
- Maintain accurate, timely data entry and documentation within the electronic health record (EHR) to support care coordination, compliance, and reporting requirements.
Who You Are Required Qualifications:
- Minimum of a High School diploma
- Completion of a Medical Terminology course preferred
- Medical office experience preferred
- Five years of work experience preferred
- Ability to pass background check upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and lower search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen.

Competencies
- Exceptional verbal and written Communication skills
- Understanding of medical terminology
- Strong work history
- Working knowledge of computers and ability to manage data within an electronic system
Working Conditions:
- High-speed internet service (cable or fiber optic) with minimum download Speed of 20 Mbps, Upload Speed of 5 Mbps, and Maximum Latency of 100 milliseconds (must be installed before starting) required.
- Frequent use of computer and phone systems
- Must be able to constantly remain in a stationary, sitting position, communicate and exchange information with others, inspect information, perform repetitive motions with arms and fingers, interpret data, problem solve, make decisions, organize and plan, and maintain a positive and professional attitude in all situations.
- Work is performed from home with company-provided equipment. Sitting for long periods of time is expected and use of fingers and hands for typing is necessary.
- A quiet workspace with minimal background noise for calls.
